Grant Regional Health Center has completed and published its latest Community Health Needs Assessment (CHNA). This assessment, conducted every three years in accordance with IRS 501(r) guidelines, aims to understand the healthcare needs of local communities.
The CHNA was a collaborative effort involving several regional healthcare organizations, including Gundersen Boscobel Area Hospital and Clinics, Crossing Rivers Health, Crawford County Public Health, Grant County Health Department, and Southwest Health. These partners share a common vision of improving community health. The process involved a community health survey for local input, analysis of secondary data from various levels, and an evaluation of community strengths and weaknesses.
The collaboration identified priority areas affecting the health and quality of life in the communities served. The goal is to better understand these needs to develop strategies that improve community health.
“Our goal is to move forward to develop new initiatives in our organization and in the community in the next three years that focus on the priorities identified through this important process,” said Tami Chambers, President & CEO. “We appreciate the time, effort and collaboration to complete this report and believe it can be a valuable resource to spark change and create healthier communities in the future.”
The 2024 CHNA highlighted Mental Health, Chronic Disease, and Substance Misuse as priority areas. Mental health emerged as the top concern among community members. Chronic conditions such as cancer, heart disease, and diabetes affect over half of Wisconsin residents. Substance misuse also poses significant challenges.
These broad areas require long-term collaboration among community organizations for improvement. Grant Regional Health Center is committed to addressing these issues by implementing internal strategies while encouraging other partners to join their efforts.
The next step involves sharing these results with the community and developing a three-year plan for health improvement strategies.
