Starting in January 2025, nearly 8,000 Family Care and Family Care Partnership (Partnership) members in South Central Wisconsin will have expanded choices for managing their long-term care. Members in Adams, Columbia, Dane, Dodge, Green Lake, Jefferson, Marquette, Rock, and Waushara counties will be able to select from four managed care organizations (MCOs) for Family Care—an increase from the two currently available. Additionally, residents in these nine counties will now have the option to participate in Partnership for the first time.

"We're giving Wisconsin Medicaid members more choices to find the care that's best for them," said Wisconsin Medicaid Director Bill Hanna. "Our Family Care and Partnership programs provide crucial care and support to help Wisconsinites live their best lives in their homes and communities."

Wisconsin's Family Care program primarily provides long-term care to older adults or individuals with disabilities. The Partnership program integrates both health care and long-term support services within home and community settings. These changes are part of a multi-year effort aimed at improving the quality and accessibility of the state's long-term care services. Maps detailing the five-year transition plan are available on the Department of Health Services (DHS) website.

Family Care and Family Care Partnership members do not need to take any action at this time. They will receive letters outlining their new options and next steps if they wish to change their MCO or program.

In January 2025, Family Care members in the nine-county region will have four MCOs to choose from:

Residents of these counties will have broader comprehensive care options:
